Job Description:

The RN Care Manager will coordinate concurrent utilization management activities house-wide and acts as the main link between the patient, health care team, organization, and payors during the patient’s hospital stay.

Responsibilities:

• Conduct assessments of patient needs and secure necessary resources for patient transition.
• Collaborate, educate, and lead an interdisciplinary team in care rounds/care conferences/care planning.
• Utilize excellent communication skills in written, verbal, and electronic systems.
• Work with community organizations to secure medical equipment, arrange transportation, and handle insurance authorizations.
• Participate in managing the length of stay for inpatient hospital populations.
• Knowledge of hospital reimbursement methods and quality measures in care management.

Requirements:

• Associate or Bachelor degree-prepared Registered Nurse with 3-5 years of clinical experience in medical/surgical nursing, home care, hospital case management, or utilization review.
• Must have experience in utilization review and discharge planning.
• Active BLS required.
• Certification by a nationally recognized case management or utilization review organization preferred (CCM or ACM preferred).
