Job Description

We are seeking a dedicated and experienced Physical Therapist to join our team, specifically for a nursing home position. The ideal candidate will have prior experience working in a Skilled Nursing Facility and possess the necessary qualifications to provide exceptional patient care.

Responsibilities

• Conduct assessments and develop individualized treatment plans for patients.
• Implement therapeutic interventions to improve patient mobility and overall health.
• Educate patients and their families regarding treatment options and home exercises.
• Collaborate with interdisciplinary teams to ensure comprehensive care for residents.
• Document patient progress and outcomes in compliance with facility policies.

Qualifications

• Valid New York State Physical Therapy license is required.
• Current Basic Life Support (BLS) certification is required.
• Previous experience in a Skilled Nursing Facility is mandatory.

Work Schedule

This position requires availability on Saturdays, with a shift from 8:00 AM to 4:30 PM.
