Job Summary

We are seeking a dedicated Social Worker to join our Clinical Resource Management team. The ideal candidate will be responsible for providing quality social work services, including discharge planning, care coordination, and care management for patients in inpatient settings.

Job Details

• Unit Type: Social Work
• Shift Length: 10 hours
• Guaranteed Hours: 40 hours per week
• Shift Days: Monday to Friday
• Shift Hours: 8:30 AM to 5:00 PM
• Weekend Requirement: Rotating
• Dress Code: Business Casual

Qualifications

• 2 years of relevant experience in social work
• Required Certifications:
  • BLS Certification (AHA)
  • NE CSW License
• Educational Requirements:
  • BSW (Bachelor of Social Work) for office roles
  • MSW (Master of Social Work) Required for inpatient Emergency Department/NICU roles

Key Responsibilities

• Conduct discharge planning and ensure seamless transitions of care for patients.
• Coordinate care management services effectively in collaboration with multidisciplinary teams.
• Assist patients in navigating social services and community resources to optimize care.
• Evaluate and assess social, emotional, and environmental needs of patients.
• Maintain accurate and timely documentation in the electronic medical record (EMR) system – Epic.

Preferred Experience

• Experience in inpatient settings, with preference for prior Emergency Room experience.

Additional Information

This position requires a valid driver’s license for commuting purposes and adherence to our fingernail policy, which prohibits artificial nails in all patient care departments.

Orientation will be provided onsite and through online platforms.
