Pharmacist – Hospital

Job Description

We are seeking a dedicated and knowledgeable Pharmacist to join our team. This full-time position involves various responsibilities aimed at ensuring optimal patient care through effective medication management.

General Duties

• Review medication orders and assess their appropriateness and completeness based on patient information including age, height, weight, diagnosis, and laboratory values.
• Contact prescribers with recommendations for improvement and resolve any identified issues with medication orders.
• Document resolutions and clinical interventions appropriately.
• Comply with Pharmacy and Therapeutics Committee directives regarding formulary management.
• Utilize clinical pharmacy skills to assess and formulate therapeutic recommendations that ensure patient safety and economic efficiency.
• Participate in decentralized pharmacist activities as appropriate.
• Review, verify, or enter medication orders into the medication management information system accurately and promptly.

Requirements

• Minimum of 1 year of experience in an acute care setting.
• Valid Pharmacy licensure.

Work Schedule

This position offers full-time hours with four 10-hour varied shifts. Weekend and holiday coverage may be required as needed.
