Ambulatory and Community Clinical Pharmacist

Job Description

The Ambulatory and Community Clinical Pharmacist will play a critical role in reviewing medication orders and evaluating their appropriateness based on patient data, including age, weight, diagnosis, and laboratory values. This position involves direct communication with prescribers to offer suggestions for medication order improvements and resolving any issues that arise.

General Duties

• Review and assess medication orders for appropriateness and completeness.
• Document clinical interventions and resolutions properly.
• Adhere to formulary management directives from the Pharmacy and Therapeutics Committee.
• Utilize clinical pharmacy skills to assess and formulate drug therapy recommendations that ensure patient safety and positive outcomes.
• Participate in decentralized pharmacist activities as applicable.
• Verify and enter medication orders into the medication management information system accurately and in a timely manner.

Minimum Requirements

• 1 year of experience in a retail pharmacy setting.
• Pharmacy licensure is required.

Work Schedule

This is a full-time position that offers flexible scheduling options, with the possibility of working four 10-hour shifts or five 8-hour shifts. Weekend and holiday availability may be required as needed.
