Staff Pharmacist

Job Description

The Staff Pharmacist is responsible for reviewing medication orders and evaluating their appropriateness based on comprehensive patient data, including age, height, weight, diagnosis, laboratory values, and other relevant information. This role involves collaborating with prescribers to enhance orders and resolving any discrepancies as needed. Clinical interventions and resolutions must be documented meticulously and in accordance with the directives of the Pharmacy and Therapeutics Committee regarding formulary management.

The Staff Pharmacist will utilize clinical skills to assess and formulate recommendations, ensuring positive patient outcomes, safety, quality, and cost-effective drug therapies. Participation in decentralized pharmacist activities, when appropriate, is also expected. This position includes reviewing and verifying medication orders, as well as entering them into the medication management information system accurately and promptly.

Minimum Requirements

• Minimum of 1 year of professional experience.
• Current Pharmacy licensure.

Job Details

Schedule: Full-time, 4 shifts of 10 hours each, varied scheduling

Weekend Requirement: As needed
