Job Description:

The Physical Therapy Assistant will provide support in home health settings, conducting visits, start of care, and evaluations. The role requires experience in home care and familiarity with Home Care Home Base.

Responsibilities:

• Conduct initial start of care, recertifications, and daily visits
• Treat an average of 6.5 patients per day
• Collaborate with a clinical manager or educator available 24/7 for emergencies

Requirements:

• Must possess a valid state license at the time of submission
• Must have home health experience
• BLS certification required
• Experience with Home Care Home Base charting system
