Palliative Care Manager (LMSW)

We are seeking a dedicated and passionate Palliative Care Manager to lead the development of a new palliative care program in Grand Rapids, MI and surrounding areas. This role provides a unique opportunity to shape a program that will have a significant impact on patients and their families facing serious illnesses. As a vital part of our mission-driven team, you will guide innovative care models while remaining deeply engaged in the essential work of supporting individuals and families during challenging times.

Key Responsibilities:

  • Lead all program startup activities, including the design of workflows, policies, and operational structures.
  • Manage the program budget and ensure financial performance.
  • Supervise and support palliative care staff in collaboration with the Medical Director.
  • Oversee compliance with Medicare, Medicaid, commercial payers, and accreditation standards.
  • Support provider credentialing and payer contracting processes.
  • Ensure efficient scheduling, referrals, documentation, and care transitions.
  • Drive program growth, customer service excellence, and performance improvement initiatives.
  • Provide social work services during startup and as required.
  • Participate in manager on-call rotation for hospice services.

Qualifications:

  • Master’s degree in social work required.
  • Preferred experience in hospice or palliative care leadership roles.
  • Prior program development experience is a plus.
  • Experience with Electronic Medical Records and Microsoft Office applications required.
  • Strong interpersonal, communication, and leadership skills, with a demonstrated ability to collaborate across disciplines and guide teams through growth and change.

Shift Information:

Typically Monday to Friday, 8:30 AM to 5:00 PM, with participation in the manager on-call rotation for hospice operations as required.

Minimum Years of Experience:

3 years required.
