Shift: 12H Days 7a-7p every Wednesday & Thursday and every other Friday, Saturday, Sunday

Job Description:

This position requires a highly experienced social worker with acute inpatient case management and discharge planning expertise. The successful candidate will have extensive experience and be capable of efficiently managing a caseload in a fast-paced environment at a large academic medical center.

Requirements:

ACTIVE License – LMSW or LCSW and ACTIVE MS State License
Minimum 2 years of Acute Inpatient Medical Case Management experience, including at least 1 year in a large hospital (500+ beds, Level 1/Academic Medical Center)
Previous travel experience in a level 1 facility within the last 3 years
Strong computer skills with proficiency in EPIC charting
Recent acute care/ED discharge planning experience within the last two years
BLS Certification
