Job Description:

Seeking an experienced Social Worker with acute inpatient case management and discharge planning experience to work closely with the case management team. Strong candidates will quickly adapt to the unit with minimal training, efficiently managing a caseload of 15-20 patients per day in a high-acuity, Level 1 trauma center environment.

Responsibilities:

• Provide acute care/ED discharge planning and coordinate complex patient placements.
• Consult with RT and Pharmacy as needed for discharge planning.
• Utilize EPIC for charting and patient documentation.

Requirements:

• Active LMSW or LCSW and MS State License.
• Minimum 2 years of acute inpatient medical case management experience in a large hospital (500+ beds, Level 1/Academic Medical Center).
• Recent travel experience in a Level 1 facility within the last 3 years.
• Strong computer skills and proficiency in EPIC charting.
• BLS Certification.
• Flu vaccine required unless medically exempt.
• Fingerprint requirement to be completed remotely or in-person the week prior to start.
