Job Description:

The social worker will be responsible for providing comprehensive psychosocial support and care coordination within an acute care setting. The role requires effective collaboration with healthcare teams to ensure positive patient outcomes and transitions of care.

Requirements:

• Must have prior California Dignity Health experience.
• LCSW and BLS required.
• Must be 5150 certified in California.
• Two supervisory references required at submittal; one within 12 months and one within 3 years worked.
• Candidates may not have been directly employed by any facility associated with CommonSpirit within the past year.
