Job Description:

The clinical pharmacist is responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for patients. They use advanced knowledge, skills, and professional judgment and serve as a patient advocate within a multidisciplinary team, adhering to all regulatory and medication safety guidelines.

Responsibilities:

• Verifies Computerized Physician Order Entry (CPOE) orders for accuracy and intervenes when orders are unclear.
• Prepares and dispenses medications, including sterile and chemotherapy preparations safely according to USP 797 & 800 guidelines.
• Executes all drug therapy management protocols, including pharmacokinetics and antimicrobial stewardship.
• Issues and maintains records of controlled substances and follows corporate policies.
• Detects and reports adverse drug reactions and medication errors promptly.
• Supervises and directs pharmacy support personnel and verifies their activities.

Requirements:

• PharmD or Bachelor’s degree with recent acute care hospital experience.
• Current pharmacist licensure in Florida.
• Experience in pharmaceutical calculations and PK dosing of Aminoglycoside and Vancomycin.
• Ability to solve issues with Pyxis and TPN calculations.
