Qualifications

• Licensure: Virginia State Pharmacist License required
• Experience: More than 2 years of hospital experience
• Technical Skills:
  • Experience with complex patient cases such as Trauma, Burn, Critical Care, Pediatrics, Cardiac Surgery, and ECMO
  • Dose Edge IV Experience
  • Experience with Pyxis
  • Proficiency in Meditech and Vigilanz
• Advanced Skills:
  • Vancomycin AUC dosing experience
  • Aminoglycoside dosing
  • Renal dosing, warfarin, enoxaparin, electrolyte protocols
  • Heparin, DOAC management
• Key Procedures:
  • IV Verification
  • Medication Verification
  • IV Compounding as needed
