Pharmacist

Job Description

The Pharmacist is responsible for reviewing medication orders and evaluating their appropriateness and completeness based on various patient data points. This includes age, height, weight, diagnosis, laboratory values, and other pertinent information. The role involves contacting prescribers with suggestions for improvement and resolving issues with medication orders as necessary. Accurate documentation of resolutions and clinical interventions is essential.

General Duties

• Comply with directives of the Pharmacy and Therapeutics Committee regarding formulary management.
• Utilize clinical pharmacy skills in assessing and making recommendations to ensure positive patient outcomes, safety, quality, and economical drug therapy.
• Participate in and support decentralized pharmacist activities when applicable.
• Review and verify or enter orders into the medication management information system in an accurate and timely manner.

Minimum Requirements

• 1 year of experience in a retail setting.
• Active Pharmacy licensure.

Work Schedule and Requirements

• Shift length includes 4 varied 10-hour shifts or 58-hour shifts.
• Weekend and holiday requirements are as needed.
