Job Description:

The role involves assessing patients to determine mental, physical, and spiritual needs, as well as planning, implementing, and evaluating an individual care plan in collaboration with the multidisciplinary health care team. Provides explanations and instructions to the patient/responsible person and uses nursing clinical judgment to recognize and report changes in patient condition.

Responsibilities:

• Assess patients to determine mental, physical, and spiritual needs.
• Collaborate with the multidisciplinary health care team to plan, implement, and evaluate an individual plan of care.
• Provide explanations and instructions to patients/responsible persons.
• Use nursing clinical judgment to recognize and report changes in patient condition.
• Contribute to a healthy workplace environment through professional communication.
• Meet Core Measure responsibilities related to patient care.
• Participate in continuing education and maintain complete and accurate medical records.

Requirements:

• Current Illinois Registered Nurse license.
• Current CPR/BLS Card.
• ACLS certification.
