Long Term Care (LTC) Registered Nurse (RN)

Job Summary

We are seeking a dedicated Registered Nurse (RN) for our Long Term Care (LTC) unit. The ideal candidate will be responsible for providing exceptional nursing care to residents, ensuring their health and well-being in a supportive environment.

Shift Information

• Shift Length: 12 hours
• Guaranteed Hours: 36 hours per week
• Scheduled Days: Every other weekend, Days
• Holiday Requirements: Every other major holiday

Unit Information

• Unit Type: Long Term Care/Special Needs Facility
• Number of Beds/Residents: 26 beds
• Electronic Medical Records (EMR): Matrix
• Nurse-to-Patient Ratios: 1:10 during the day, 1:19 at night
• Common Diagnoses: Heart disease, diabetes, dementia

Key Responsibilities

• Serve as a charge nurse, overseeing care on the unit.
• Administer medications and perform wound care.
• Coordinate resident care with the interdisciplinary team as needed.

Qualifications

• Experience: Minimum of 2 years in Long Term Care (LTC) settings.
• Certifications: BLS Certification required.
• Skills: Strong clinical skills and the ability to work effectively in a team environment.

Additional Information

• Orientation: 2-3 shifts of unit-specific orientation provided.
• Physical and Emotional Requirements: Candidates should be able to provide emotional support to residents and their families.
• Support Staff: Limited support staff available; social worker designee for resident needs.
