Job Description:

The Hospice teams support patients and family members through the final stages of their illness. Services are offered in home settings and may include the patient’s own residence, skilled nursing facilities, and more. Services include intermittent skilled nursing visits, pain and symptom management, social services, counseling, spiritual support, and bereavement support. Medications and equipment are provided as needed.

Requirements:

• Current unencumbered Oregon RN Licensure required upon submission.
• Healthcare Provider Level BLS required.
• BSN and/or previous HH/Hospice or case management experience preferred.
• Previous broad-based nursing experience preferred.
• Valid driver’s license and auto liability/property damage insurance required.
