Home Health Registered Nurse

Job Description

A Home Health Registered Nurse is responsible for providing patient care in the comfort of their own homes. This position involves conducting assessments, developing care plans, and managing patient needs effectively.

Responsibilities

• Perform OASIS assessments for in-home patients.
• Develop and implement individualized care plans.
• Provide comprehensive nursing care and support to patients in their homes.
• Participate in the department’s call rotation, which includes responding to patient needs during nights, weekends, and holidays.

Shift Information

Day shifts are scheduled from Monday through Friday, with a call rotation of one in four for nights, weekends, and holidays.

Qualifications

• Minimum of 3 years of nursing experience, with at least 2 years of experience in home health care.
• Experience performing OASIS assessments and care planning for in-home patients is required.
• Familiarity with Epic software is a plus.

Work Schedule

Full-time positions available, with an expectation of 32-40 hours per week.
