Shift: 8 Hour Days, Monday-Friday 8am-5pm with on-call every 3rd weekend and 1 night per week

Contract Length: 13 weeks

Job Description:

The role involves providing home health services to patients in Nelson, Spencer, and southern Bullett counties. Clinicians will call patients to schedule visit times and provide care focused on home health with emphasis on Start of Care, recerts, and daily visits. This position requires use of the Homecare Homebase (HCHB) charting system and a valid Driver’s License & Insurance.

Responsibilities:

• Conduct Start of Care, recertifications, and daily visits for home health patients.
• Coordinate with multidisciplinary team for effective patient care.
• Participate in on-call rota one night per week and every 3rd weekend.
• Document patient care using Homecare Homebase (HCHB).

Requirements:

• Must have experience in Hospice and with Homecare Homebase (HCHB).
• BLS Certification required.
• Valid Driver’s License & Insurance are essential.
• Experience with pediatric and adult patient demographics.
