Job Description:

The Registered Nurse will provide home health care services to adult patients in the Elizabethtown area, covering Marion, Washington, Taylor, LaRue, Hardin, Meade, and Breckenridge Counties. The role involves managing patient care using the Homecare Homebase charting system and requires home health experience.

Responsibilities:

• Manage a caseload of approximately 30 patients with a goal of achieving 6.5 points per day.
• Perform start of care assessments, recertifications, and daily visits.
• Provide clinical care and coordinate treatment plans.
• Ensure accurate documentation in the Homecare Homebase system.

Requirements:

• Must have 1 year of experience on a Medsurg floor in a hospital setting.
• Licensed in the state of Kentucky.
• BLS Certification required.
• Experience with Homecare Homebase charting system.
