Job Title: RN (Registered Nurse), RN, Home Health

Shift: 8 hour days + weekend rotation

Job Description:

This position involves providing home health services in the coverage area of Nelson, Spencer, and southern Bullett counties. The RN will efficiently manage approximately 30 patients, completing start of care, recertifications, and daily visits with a requirement of 6.5 points per day.

Responsibilities:

• Manage and provide care to adult patients only, utilizing the Homecare Homebase (HCHB) charting system.
• Coordinate patient care with a clinical manager or educator available for support 24/7.

Requirements:

• Must have Homecare Home Base experience and a background in home health.
• Licensed RN in Kentucky.
• At least 1 year of experience on a Medsurg floor in a hospital setting.
• BLS (AHA) certification required.
