Job Description:

Call requirements are every 6-7 weeks for one week at night from 5pm-8am and that weekend. We do not make night visits from this office.

Responsibilities:

  • Provide care to 5-6 patients per day in a home health setting
  • Document patient care using HCHB system

Requirements:

  • Home health and hospice experience required for nursing roles
  • Reliable vehicle for patient visits
