Home Health Registered Nurse (RN)

We are seeking an experienced Home Health Registered Nurse (RN) to join our team. This position requires a strong background in home health and home care charting.

Coverage Area

The coverage area for this position includes Marion, Washington, Taylor, LaRue, Hardin, Meade, and Breckenridge Counties.

Key Responsibilities

• Conduct start of care assessments and re-certifications.
• Perform daily visits to adult patients, managing a caseload of approximately 30 patients.
• Coordinate care and communicate effectively with patients, families, and healthcare teams.
• Ensure compliance with all regulatory and organizational policies.

Qualifications

• Must have a minimum of 1 year of experience on a Medical-Surgical floor in a hospital setting.
• Home care and home base charting experience are essential.
• Must be licensed in the state of practice.

Additional Information

• Typical shifts are 5 days of 8-hour shifts.
• Weekend shifts may be required based on nurse rotation.
• Participation in holiday rotation is expected.
• No on-call responsibilities.
• Scrubs are required (no specific color requirement).
• Orientation includes 2 weeks of training with a preceptor.
• A phone and tablet will be provided for work purposes.
• Support is available from a clinical manager or educator 24/7 for emergencies and from 8 AM to 5 PM for general inquiries.
