Job Description:

This position involves facilitating transfers of any patient type from hospitals within and outside of Dignity Health. Responsibilities include patient assessments, gathering clinical information, and coordinating with accepting physicians and specialists. The role requires strong critical care assessment capabilities and the ability to efficiently manage multiple tasks simultaneously.

Responsibilities:

• Facilitate transfers of patients after the intake process.
• Conduct patient assessments and gather vitals and clinical information.
• Coordinate with MDs and specialists, utilizing hospital services lists for bed requests.
• If LVN, focus on referring out to other facilities when necessary.

Requirements:

• MUST have a minimum of 2 years of ICU, ED, or transfer center experience.
• MUST have acute care experience and critical care assessment skills.
• MUST be proficient in using multiple computer applications including Cerner, TeleTracking, and Google apps.
• MUST possess ACLS (AHA), BLS (AHA), and NIHSS certifications.
• Ability to work holidays and weekends according to schedule needs.
