Job Description:

Pagosa Springs Medical Center is seeking two experienced Registered Nurse travelers to join our Rural Health Clinic team. The Clinic RN serves as a key member of a team-based primary care model, collaborating closely with physicians, nurse practitioners, physician assistants, medical assistants, LPNs, and support staff to provide high-quality outpatient care. The RN assists with patient triage, care coordination, patient education, medication management, chronic disease management, and clinical support services while helping ensure an exceptional patient experience.

Responsibilities:

  • Provide comprehensive nursing care within the outpatient clinic setting.
  • Perform patient assessments, triage, care planning, treatment implementation, and follow-up.
  • Manage patient phone calls, portal messages, and nursing advice using established protocols.
  • Administer medications, injections, immunizations, and other clinic-based treatments.
  • Perform point-of-care testing and CLIA-waived laboratory procedures.
  • Coordinate care with providers, specialists, and ancillary departments.
  • Complete accurate and timely documentation in the electronic health record.
  • Support quality improvement initiatives and patient engagement efforts.
  • Serve as a clinical resource for patients and clinic staff.

Requirements:

  • Current Colorado RN license or active compact RN license in good standing.
  • Current AHA BLS certification.
  • Minimum two (2) years of recent clinical nursing experience.
  • Cerner EMR experience required.
  • Strong assessment, triage, patient education, and care coordination skills.
  • Ability to work independently in a fast-paced outpatient clinic environment.
