Job Description:

The RN Case Manager will work across all inpatient units providing acute inpatient case management and discharge planning services. The role requires the candidate to have strong computer skills, Epic EMR experience, and the ability to work efficiently with minimal training.

Responsibilities:

• Manage a case load of 15-20 patients per day.
• Collaborate with the social work team and other departments as needed for discharge planning.
• Perform acute inpatient case management and discharge planning.

Requirements:

• Minimum 3 years of acute inpatient case management experience, including at least 1 year in a large hospital (500+ beds, Level 1/Academic Medical Center).
• Experience in a Level 1 trauma facility is required.
• Strong computer skills with Epic EMR experience.
• BLS certification through AHA is required, CCM is preferred but not mandatory.
• Nursing degree preferred RN, BSN; would consider ADN.
