• Number of Beds: 40
• Staffing: 1-2 Case Managers or Social Workers
• Patient Ratios: 1:20-30
• Electronic Medical Record (EMR) System: Meditech, Careport
• Floating Requirement: Varied Case Manager assignments

Job Description

We are seeking a Case Manager (RN) with experience in acute care settings. The ideal candidate will have a strong background in case management and a collaborative approach to patient care. Candidates that have familiarity with Meditech and Careport systems are preferred.

Responsibilities

• Provide comprehensive case management services for patients in the Neuro Step Down unit.
• Collaborate with healthcare team members to develop and implement effective patient care plans.
• Facilitate communication between patients, families, and healthcare providers.
• Assist in transitioning patients to appropriate care settings post-discharge.

Qualifications

• Minimum of 1 year of acute care case management experience.
• Preferred: Knowledge of Meditech and Careport systems.
• Preferred: Certification in Case Management (CCM).
