Job Description

We are seeking a dedicated and qualified Case Manager (RN) to join our team. The ideal candidate will possess strong acute care case management experience and be able to work effectively in a dynamic environment. This position involves collaborating with interdisciplinary teams to ensure optimal patient care and efficient healthcare delivery.

Key Responsibilities

• Provide acute care case management services for patients within the neuro step-down unit.
• Coordinate and collaborate with healthcare providers to optimize patient outcomes.
• Monitor patient progress and adjust care plans as necessary.
• Facilitate communication between patients, families, and healthcare team members.
• Document case management activities in the EMR system.

Qualifications

Required:

• Minimum of 1 year of acute care case management experience.

Preferred:

• Experience with Meditech and Careport EMR systems.
• CCM certification.

Work Schedule

This role typically requires working day shifts from 8:00 AM to 4:30 PM. Specific scheduling may vary based on unit needs.
