Job Description

We are seeking a dedicated and experienced Registered Nurse (RN) with a focus on Care Management. This role is essential in ensuring optimal patient care and effective case management within our healthcare team.

Schedule

• Day Shift: 8:00 AM – 4:30 PM
• Days: Tuesday to Saturday
• Hours: 40 hours per week

Responsibilities

• Manage a heavy caseload of patients, providing comprehensive care management services.
• Coordinate treatment plans and facilitate communication among multidisciplinary teams.
• Assess patients’ needs and develop effective care strategies.
• Utilize case management best practices to enhance patient outcomes.

Qualifications

• Education: Bachelor’s Degree in Nursing (BSN) is preferred.
• Experience: Prior experience in a Care Management role is preferred, specifically with experience in inpatient care management (IP CM).
• Licenses/Certifications:
  • Current Registered Nurse (RN) licensure in the state of practice is required.
  • Case management certification is preferred.
  • Basic Life Support (BLS) certification from an approved vendor per AH policy is required.

Preferred Qualifications

• Experience with strike force programs for KP or HCA in California is preferred.
• Ability to handle a high volume of cases independently with minimal training.
