Registered Nurse (RN)

Job Description

We are seeking a qualified and experienced Registered Nurse to join our care management team. The successful candidate will provide high-quality care management services, working collaboratively with healthcare professionals to ensure optimal patient outcomes.

Schedule

• Day Shift: 8:00 AM – 4:30 PM
• Days: Sunday to Thursday
• Total Hours: 40 hours per week

Key Responsibilities

• Manage a heavy caseload of patients effectively and efficiently.
• Implement care management strategies and interventions based on individual patient needs.
• Conduct comprehensive assessments, develop care plans, and collaborate with interdisciplinary teams.
• Utilize experience in care management practices, particularly in inpatient settings.
• Participate in quality improvement initiatives and contribute to patient safety efforts.

Qualifications

• Education: Bachelor’s Degree in Nursing (BSN) preferred.
• Experience: Previous experience in a care management role is preferred. Familiarity with strike force operations for healthcare organizations is a plus.
• Licenses/Certifications:
• Registered Nurse (RN) licensure in the state of practice is required.
• Case management certification is preferred.
• Basic Life Support (BLS) certification from an approved vendor per organizational policy is required.

Additional Information

This position does not require on-call duties. We are looking for candidates who can adapt quickly and thrive in a fast-paced environment with minimal training. Candidates with a background in inpatient case management will be highly regarded.
