- Number of Beds: 289
- Number of Staff: 23 (Full-Time and PRN)
- Staff Composition: Mix of RNs and Social Workers
- Day Shift Staffing: 10-13 RNs and Social Workers
- Night Shift Staffing: 0
- Patient Ratios: RN 1:25, Social Worker 1:40
- Documentation Systems: Meditech, Midas, Wellsky
General Information
Role Responsibilities
- Provide case management services in an acute care setting.
- Ensure effective patient care coordination and discharge planning.
- Collaborate with a multidisciplinary team to meet patient needs.
