• Patient Ratios: Minimum of 25:1 for Case Management
• EMR Systems: Meditech, Midas

Responsibilities

• Assume responsibility for patient evaluation and complex discharge planning.
• Coordinate patient care and communicate effectively with interdisciplinary teams.
• Utilize Midas and Meditech for documentation and case management activities.
• Provide clinical support and guidance to staff members.

Qualifications

• Required Skills:
• Experience with Midas and Meditech.
• Knowledge in complex discharge planning and patient care management.
• Preferred Skills:
• Experience in a case management role within a hospital setting.
• Strong communication and organizational skills.
