Job Description:

The Registered Nurse Case Manager will be responsible for care coordination and case management, ensuring efficient discharge planning and utilization review within an acute care setting.

Responsibilities:

• Manage a 1:20 patient assignment ratio in care coordination.
• Cover all adult areas of the hospital, except women’s health if lacking experience.
• Participate in 4 days of webinar charting orientation and 2 days with a preceptor.

Requirements:

• Must have prior experience with Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs.
• Minimum of 3 years hospital experience with a strong background in acute care coordination and discharge planning.
• Must possess BLS Certification.
