Job Description:

The RN Case Manager will be responsible for care coordination and case management within an acute care setting. The role requires proficiency in Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs, with strong experience in discharge planning and utilization review.

Responsibilities:

• Manage patient assignments with a 1:20 ratio.
• Participate in floor-specific orientation, including webinars and preceptorship.

Requirements:

• Minimum of 3 years hospital experience with acute level care coordination experience.
• Proficiency with Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs.
• BLS Certification required.
