Job Description:

This is a hybrid role responsible for the active management of assigned patients and serving as a clinic nurse. Collaborates with the medical provider and interdisciplinary team members to establish individualized medical goals. Coordinates care conferences for high-risk or complex patients and manages acute and chronic care needs. Facilitates patient referrals and ensures follow-up, acting as a liaison for case managers and discharge coordinators.

Responsibilities:

• Collaborate with medical providers to maximize patient healthcare outcomes.
• Coordinate multi-disciplinary care conferences.
• Manage acute and chronic care needs, reviewing EHR reminders and providing outreach and education.
• Participate in case management reviews for high-need patients.
• Facilitate coordination of patient referrals and track outcomes.
• Perform nursing duties including assessment, documentation, and administering medications.
• Maintain a clean and safe working environment and adhere to infection control policies.
• Participate in the continued development of the case management role in primary care.

Requirements:

• Graduate of an accredited school of nursing; Bachelor’s degree preferred.
• Current full, unrestricted Registered Nursing license in Alaska.
• Basic Life Support certification.
• At least 1 year of case management experience within the last 2 years.
• Knowledge of safety and infection control principles.
• Skills in assessment, anticipation of needs, and data collection.
• Ability to multi-task and work independently in a fast-paced environment.
