Job Description:
The Physical Therapist will provide home health care services to patients in Southern Santa Fe and Northern Albuquerque, ensuring the delivery of comprehensive rehabilitation in the home setting.
Responsibilities:
- Cover Southern Santa Fe and Northern Albuquerque areas.
- Provide home health care and in-home hospice care, achieving 30 points per week.
Requirements:
- Must have home health experience utilizing Homecare Homebase.
- Experience performing start of cares (SOC) is required.
