Job Title: PT (Physical Therapist), Rehab

Shift: 8H Days M-F, 8a-5p + rotating weekends

Contract Length: 13 weeks

Job Description:

Provide physical therapy services in a home health setting, performing evaluations, recertifications, and regular visits to patients with various diagnoses including CHF, ORTHO, and pneumonia.

Responsibilities:

• Perform “start of care” assessments, recertifications, and evaluations.
• Work collaboratively with a clinical manager or educator who is available 24/7 for emergencies.
• Use the Homecare Homebase (HCHB) charting system.
• Follow a rotating schedule including some weekends and holiday rotations.

Requirements:

• Must have Home Care Home Base experience and home health experience.
• Must have a valid license in hand upon submitting.
• BLS (AHA) certification required.
• Prefer candidates with some professional experience (no new grads preferred).
