Job Description:
Provide physical therapy evaluations, establish plans of care, and assist with rounds and care conferences in an inpatient/outpatient setting.
Responsibilities:
- Perform physical therapy evaluations and establish plans of care.
- Assist with rounds and participate in care conferences as indicated.
- Communicate frequently with the rehab team regarding patient progress, needed equipment, and discharge needs.
Requirements:
- Experience in hospital and/or nursing home setting.
- Must have CPR certification and NC Physical Therapy license.
