Job Description:

The Physical Therapist will provide care to home health and hospice patients within a 60-mile radius around Valley City, ND. Responsibilities include managing a caseload of 5-7 patients per day with a focus on rehabilitation following surgeries and falls.

Responsibilities:

• Provide physical therapy to home health and hospice patients.
• Manage a caseload effectively, addressing rehabilitation needs.
• Document treatments and patient progress in the HomeCare/HomeBase system.

Requirements:

• BLS certification.
• North Dakota Physical Therapy License.
• Current Driver’s License & Auto Insurance.
• 1+ year of physical therapy experience with previous home health experience.
