Job Description:

The Physical Therapist will provide physical therapy services to patients in a skilled nursing facility setting, focusing on improving patient outcomes and promoting mobility.

Responsibilities:

• Assess patients’ physical conditions and develop appropriate treatment plans.
• Implement therapy programs and conduct regular evaluations to track progress.
• Work collaboratively with other healthcare professionals to ensure quality patient care.
• Maintain accurate and up-to-date patient records.

Requirements:

• Current licensure as a Physical Therapist in the state of Wyoming.
• SNF experience required.
• Experience working in long-term care or nursing home settings preferred.
