Job Description:

The Physical Therapist (PT) provides treatments in various settings, including homes, Assisted Livings, Senior Apartments, and Board and Care homes. The PT is responsible for managing a caseload of 3 to 5 patients per day, ensuring travel, documentation, and case management are efficiently handled.

Responsibilities:

• Evaluate and treat patients with varied diagnoses except for pediatrics or Hospice.
• Coordinate care with territory-based teams to reduce travel time.
• Supervise up to two PTA/COTAs.
• Complete all work by the end of the scheduled shift, with minimal monthly weekend work required.
• Participate in a comprehensive 3-week orientation and begin solo work by the end of the 3rd week.

Requirements:

• Valid CA PT license and BLS certification required.
• 2 years of experience in a healthcare setting, with home health experience preferred.
• Experience with EMR systems, preferably HomeCare Home Base (HCHB).
• Current driver’s license and proof of automobile insurance.
• Strong oral and written communication skills with good organizational abilities.
