Job Title: PT (Physical Therapist)

Shift: 8:30a-5:00p, typically one Saturday a month

Job Description:

Provides physical therapy services to patients/clients according to a written physician’s plan of care, including assessing, evaluating, updating therapeutic/rehabilitative/functional status, and participating in the development of the total plan of care.

Responsibilities:

• Assess, evaluate, and update therapeutic/rehabilitative/functional status.
• Evaluate home environment and assess for muscle strength, mobility, gait, and ROM potential for rehab.
• Instruct patients/clients and families/caregivers in the use and care of therapeutic appliances.
• Report the patient’s/client’s reaction to treatment or changes in condition to the physician.
• Initiate physical therapy program and instruct others in certain phases.
• Supervise homemaker/Home Health Aides when they participate in patient/client rehabilitation.
• Prepare and submit clinical and progress summaries and report patient condition to the attending physician.
• Provide physical therapy consultation to home families/caregivers.
• Participate in peer consultation process and complete ongoing staff competency assessment.
• Comply with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and other regulations.

Requirements:

• Minimum of 2 years experience as a PT in an acute/home care/rehab setting.
• BLS Certification required.
• Good oral and written communication skills; organizational skills.
• Dress code: scrubs are not required. If scrubs are not worn, dress code is slacks (khakis are fine) and polo or button-up shirt.
• BSN not required. License to submit: Yes.
