Job Description:

Seeking a Physical Therapist with home health experience and Home Care Home Base proficiency to serve four counties primarily consisting of rural areas with mid-sized towns. Must perform Oasis Assessments and Start of Care cases, typically handling 4-6 patient visits daily.

Responsibilities:

• Conduct patient visits and manage a caseload using a tablet to track time and visits
• Perform Oasis Assessments and Start of Care cases

Requirements:

• Must have home health experience and Home Care Home Base experience
• License in hand upon submitting
• Required Certification: BLS (AHA)
