Job Description:

The Licensed Practical Nurse (LPN) will provide care to residents in a long-term care, rehabilitation, and dementia setting. The role requires flexibility to work variable shifts, primarily during the day, and involves direct patient care, documentation, and collaboration with a multidisciplinary team.

Responsibilities:

• Provide nursing care to residents in accordance with established standards and regulations.
• Document care provided in the electronic medical record system (PCC).

Requirements:

• Valid LPN license in the state of Vermont.
• Experience in long-term care, rehabilitation, or dementia settings preferred.
