Social Worker – Social Worker Case Manager

Job Description:

SW INPT (inpatient) will also be cross-trained for ED. They will predominantly work the 10s: inpatient 8-6:30. They will be backup for the ED with advance notice. The schedule for the entire 13 weeks will be built and given on their first day.

Responsibilities:

• Acute care case management experience is a must
• Behavioral health assessment, diagnosis, treatment planning, discharge planning for post-acute placements
• Could also be the remote resource Social Worker for the Off Campus EDs in the area

Requirements:

• Master’s in social work and WA state License
• Emergency Room Assignment: Licensed Independent Clinical Social Worker (LICSW) is required
• Inpatient Unit: LICSW preferred; minimum of Licensed Social Work Associate Independent Clinical (LSWAIC) required with proof of current supervision status needed
• EPIC experience required
• Minimum 2 years of recent acute inpatient social work/case management experience and 4 years of overall social work experience
