Job Description:

The role involves providing professional nursing care in coordination with the ‘Care Coordination’ team. Responsibilities include patient assessments, developing care plans, and executing interventions to optimize patient outcomes.

Responsibilities:

• Perform patient assessments and document findings.
• Develop and implement individualized care plans.
• Collaborate with interdisciplinary teams for comprehensive care delivery.
• Float to different units as needed.

Requirements:

• Prior CA Dignity experience is mandatory.
• LCSW and BLS certifications required.
• Must be 5150 certified in California.
• Two supervisory references needed at submittal, one within the last 12 months and one within the last 3 years.
• Candidates must not have been directly employed by any facility associated with CommonSpirit within the past year.
