Job Description:

RN – Manager responsible for collecting and analyzing patient data to determine diagnosis and care issues, developing and implementing care plans, collaborating with patients, families, and healthcare providers to deliver safe and compassionate care, and evaluating progress toward outcomes.

Responsibilities:

• Collect relevant data pertinent to the patient’s health or situation.
• Analyze assessment data to determine diagnosis and care issues.
• Develop a plan prescribing interventions to attain outcomes.
• Implement the plan and coordinate care delivery.
• Evaluate progress toward attaining outcomes.
• Collaborate with the team of patient, family, and healthcare providers.
• Provide learning opportunities for patients/family members and team members.
• Participate in discharge planning.
• Delegate appropriately and coordinate duties of healthcare team members.
