Job Description:

As a Registered Nurse in Home Health, you will conduct initial and continuous client assessments to determine care needs, plan and revise care based on diagnosis, administer medications, and instruct clients per the care plan. You will also coordinate with physicians and prepare clinical notes, functioning as Case Manager when assigned.

Responsibilities:

• Initial assessment of client and family to determine home health needs, complete physical assessment & history of current & previous illness.
• Regularly re-evaluate client nursing needs and determine nursing diagnosis using health assessment data.
• Initiate and revise plan of care as client status & needs change; include client/family in planning.
• Administer medications and treatments as prescribed by the physician.
• Provide health care instructions to the client as per assessment and plan of care.
• Identify discharge planning needs and implement as part of care plan development prior to discharge.
• Act as Case Manager when assigned and coordinate client care for assigned caseload.
• Prepare clinical notes and update the primary physician when necessary.
• Communicate with the physician regarding client needs and report changes in condition; obtain physician orders as required.
• Ensure arrangements for equipment and other necessary items and services are available.
• Instruct, supervise, and evaluate home health aide care provided.

Requirements:

• Home Health and Hospice experience is required.
• Current Driver’s License & Auto Insurance.
• BLS Certification.
