Job Description:

Registered Nurse needed for Home Health and Hospice care, focusing on adults, geriatrics, and pediatrics. Must have Home Health experience and OASIS proficiency. Responsibilities include completing assessments, developing care plans, administering medications, coordinating care, and instructing home health aides. Must provide care within a 60-mile radius around Dickinson.

Responsibilities:

  • Initial assessment of client and family to determine home health needs – complete physical assessment & history of current & previous illness.
  • Regularly re-evaluates client nursing needs. Uses health assessment data to determine nursing diagnosis.
  • Initiates plan of care, makes necessary revisions as client status & needs change.
  • Develop care plan/establish goals based on diagnosis & incorporate therapeutic, preventive, rehabilitative nursing actions. Include the client/family in planning process.
  • Administers medications and treatments as prescribed by the physician.
  • Provides health care instructions to the client as appropriate per assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate client care for assigned caseload.
  • Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.
  • Communicates with the physician regarding the client’s needs and reports any changes in the client’s condition; obtains/receives physician’s orders as required.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Instructs, supervises and evaluates home health aide care provided.

Requirements:

  • Must have Home Health & Hospice experience.
  • Must have current Driver’s License & Auto Insurance.
  • Certifications Required: BLS
  • Scrub Color: Any solid color scrubs
