Registered Nurse (RN)

Job Description

We are seeking a dedicated and experienced Registered Nurse (RN) to provide exceptional home health care services in Fairbanks, Alaska. This position involves working closely with patients in their homes, ensuring they receive the highest quality of care in a supportive and compassionate environment.

Work Schedule

• Start Date: 10/12 or 10/26
• Initial Schedule: Monday day shift (8 hours), with the rest of the schedule to be determined during a supervisor meeting on Monday
• Typical Hours: 8:00 AM – 4:30 PM, Monday to Friday
• Weekend Requirement: On-call coverage required, averaging one weekend per month
• Overnight Requirements: 1-2 days per week from 4:30 PM to 8:00 AM
• Holiday Coverage: Rotating holiday coverage required

Responsibilities

• Provide high-quality nursing care to patients in their homes.
• Conduct comprehensive assessments and evaluations of patients’ health.
• Develop and implement individualized care plans that meet patients’ needs.
• Collaborate with interdisciplinary teams to optimize patient care.
• Educate patients and their families about health management and treatment options.

Qualifications

• Required: Minimum of 2 years’ experience in CMS/OASIS Home Health program
• Preferred: At least 5 years of home health experience
• No new graduates accepted
• Must possess a valid nursing license and any necessary certifications
• Must have reliable transportation for travel to patient homes
