Case Manager (RN)

Position Overview

The Case Manager (RN) plays a vital role in ensuring comprehensive patient care through effective case management practices. This position involves collaborating with patients, families, and healthcare teams to facilitate optimal treatment and discharge planning.

Unit Information

Typical Census: 180-230 patients including Women & Children
Number of Case Managers: 6 total
Staffing Structure: 1 CM weekend option (Sat/Sun, 7a-7p) and 4-5 CMs on weekdays (Mon-Fri, 8a-4:30p)
Patient Ratios: 1:40 on weekdays
Equipment: Laptops and cell phones
EMR: MediTech and Midas

Key Responsibilities

• Conduct face-to-face interviews with patients and families.
• Perform daily Medical Discharge Reviews (MDRs) on each unit.
• Conduct InterQual reviews for every patient along with observation reviews, Discharge Planning Evaluations (DPEs).

Qualifications

• Required Skills:
  • Case Management experience.
  • Proficiency in Midas and InterQual.
  • Active Licensed Masters degree.
• Preferred Skills:
  • Knowledge of community resources.
  • Experience with NATE.

Rotation and Dress Code

• Weekend Rotation: Saturday and Sunday, 8a-4:30p

This position may require floating to various departments within the hospital.
