Case Manager (RN)

Position Overview

We are seeking a skilled and dedicated Case Manager (RN) to join our team. This role is crucial in ensuring effective patient care and managing discharge planning in a fast-paced clinical environment.

Unit Information

• Number of Beds: CPCU – 14 beds, PCU – 14 beds
• Patient Ratios: 1:14
• Type of Equipment: EMR – Meditech and Midas

Schedule

This position offers a day shift schedule from Monday to Friday, 8:00 AM to 4:30 PM. No weekends are required.

Qualifications

Required Skills:

• Minimum 1 year of current acute care Case Manager RN experience
• Basic Life Support (BLS) certification
• Active RN licensure in Kansas

Preferred Skills:

• Experience with Midas and HCA case management
• Prior experience in a healthcare system

Responsibilities

• Conduct case management assessments for patients.
• Develop and implement effective discharge planning processes.

The CPCU unit specializes in cardiac step-down care, where patients recovering from CABG and TAVR procedures are typically placed. The PCU unit provides step-down medical care for a diverse range of patients with various needs.
